Location: 2666 Riva Road, Suite 400, Annapolis, MD 21401
Salary: $29.99 per hour with benefits or $32.62 without benefits
Benefits: Participation in the County’s group health plan (including prescription, dental and optical), paid holidays, annual leave and sick leave.
Nature of Work: To perform in-home assessments with adults who are disabled, older/frail. Perform STEPS evaluations with client and caregiver in the home. Compose a plan of care for each client. The plan of care consists of a brief medical and psychosocial history, current medications, current functions and other pertinent issues. It also contains recommendations and referrals, options available to the client and the outcome. The human services evaluator will also make appropriate referrals to various programs. Clear and concise documentation is vital. The human services evaluator must enter the information in a database and maintain a file.
Experience: Two or more years of professional experience in human services program areas. Long-term care and community experience preferred.
LICENSE: Must possess a current Maryland license as a registered nurse from the Maryland Board of Nursing or licensed social worker (LCSW-C).
Note: The applicant must be able to work independently, be knowledgeable of community resources, have the ability to travel through the county and be computer literate. Candidates with experience in community health/long-term care are encouraged to apply.
