Job Description
Nature of Work: Provide comprehensive care coordination and case management services for individuals residing in recovery residences. The goal of this role is to assess clients' safety and stability, promote independence, and support individuals in their transition to independent living within the community. This position is key in fostering individuals’ long-term recovery and self-sufficiency, aligned with the (State Opioid Response) SOR-IV program’s objective to support the continuum of recovery services. Assess and assist with care coordination for individuals placed in recovery residence Crisis Stabilization Services (CSS) beds. |
Minimum Qualifications
Education: Associate degree |
Experience: Conducting screenings and assessments for substance use disorder treatment with an understanding of ASAM levels of care and DSM-5 diagnostic criteria for opioid use disorder. Experience providing care coordination and case management services. |
License or Certification: Good standing as a Licensed Masters Social Worker (LMSW), Licensed Graduate Counselor (LGPC), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Alcohol and Drug Counselor (LCADC) or Licensed Clinical Social Worker–Certified (LCSW-C). Certification by the Board of Professional Counselors as a CAC-AD, CSC-AD and ADT will be considered. |
Notes: Must have a personal vehicle available for travel to perform work duties and a current Maryland driver’s license with a clear driving record. |
Desired or Preferred Qualifications: Experience providing care coordination and case management services. |
