Job Description

Nature of Work:

This is professional work on behalf of senior and special needs citizens and their family members. Work involves extensive contact with the public, community organizations and agencies, and county and state personnel.

Will travel throughout Anne Arundel County to homes/workplaces of the Maryland Medical Assistance Medical Care Program. Participants to verify the initial standard assessment of functional capability, termed the interRAI Home Care Assessment Form, complete a nursing assessment, evaluate the delivery of care performed by Residential Service Agency/Personal Assistance Provider Agency Staff, and determine the need for discharge from personal assistance services or referral to other services. This is determined through the following:

  • Initiating contact with the client within 10 days of being assigned new clients and documenting contact attempts in the Maryland Long Term Services and Support (LTSS) System.
  • Visits to the home of each participant assigned to him/her at least as often as the Department’s regulations mandate, but not more frequently than is clinically appropriate and collaboratively determined with the participant, to conduct an assessment of the participant (in person or telephonic) to determine his/her general health and welfare.
  • Ascertaining whether the Residential Service Agency (RSA) worker is providing the number of Personal Assistant Services (PAS) hours approved on the participant’s Plan of Service by reviewing the RSA worker’s time worked.
  • Ascertaining whether the RSA registered nurse is providing on-site supervision of the RSA worker in line with the frequency noted in COMAR 10.07.05.12 and whether the RSA registered nurse has completed an evaluation of the RSA worker.
  • Determining whether the RSA registered nurse and the RSA worker are completing the required documentation in a timely manner.
  • Engaging the participant, and/or his/her representative, if applicable, regarding whether he/she is satisfied with PAS provided by the RSA worker and the RSA generally.
  • Engaging the participant (including making observations) as to whether he/she is experiencing abuse, neglect and/or exploitation by any RSA staff.
  • Reporting to the Department any incident or complaint that is negatively impacting, or has the potential to negatively impact, a participant’s health and welfare.
  • Maintaining a clinical profile and record on each client he/she monitors, in the Maryland Long Term Services and Support (LTSS) and Tracking System, in accordance with the Department.
  • Implementing a process to evaluate the quality of nurse monitoring services provided to eligible participants.

Minimum Qualifications

Education: 

Associate degree in nursing from an accredited college or university.

Experience:

Preference will be given to those with community health home visiting experience. Must be able to work independently, be knowledgeable of community resources or willing to learn about them, and have the ability to travel throughout the county. Be comfortable conducting in-person home visits and be computer literate.

License or Certification:

Current Maryland license as a Registered Nurse from the Maryland Board of Nursing. Valid non-commercial Class C motor vehicle operator's license.

Desired or Preferred Qualifications:

Two or more years of experience in public health services or community health home visiting. Skills beyond a beginner level in data entry and Google Workspace.
