Assessment, Evaluation and Review Services (AERS)

AERS serves County residents age 18 and older who are chronically ill or disabled and at risk for institutionalization and individuals with medical assistance of any age who would like to apply for programs that provide support. Licensed clinical social workers and registered nurses perform comprehensive assessments on AERS clients in an effort to allow them to live in the safest and least restrictive environment possible, while fostering independence and personal welfare.

NMS promotes health, wellness and safety through ongoing quality monitoring of the Personal Assistance Service, so that individuals may age in place, in the community of their choice.
